What anti-wrinkle treatment actually does.

Anti-wrinkle injections use a purified botulinum toxin protein to relax the specific muscles that fold your skin every time you frown, squint or raise your brows.

Expression lines are made by movement. Every time you raise your brows or squint, the muscle underneath folds the skin above it. Do that several thousand times a year for a couple of decades and the fold stops bouncing back, which is why a line that once appeared only when you moved eventually sits there while your face is at rest.

The injection reduces how hard that muscle can contract. With the repeated folding eased, the skin above it gets a chance to soften, and lines that were becoming permanent become shallow again. It works on lines caused by movement, which is why it is excellent on the forehead, between the brows and around the eyes, and does very little for lines caused by lost volume or sun damage.

How much movement you keep is a decision, not a side effect. We dose to soften rather than to erase, because a forehead that cannot move at all reads as treated, and the brow tends to drop when the muscles that hold it up are over-relaxed. If you want a stronger result you can have one, but we would rather start conservative and top you up two weeks later than take movement away you did not want to lose.

The effect is temporary. Muscle activity returns gradually over three to six months and lines come back as it does, so this is a treatment you repeat rather than one you finish.

Why the same product gives such different results

The product is not the variable. The same toxin is available to every clinic in London, and it is neither better nor worse in our hands than anyone else's. What changes the result is which muscles are injected, how deep, how many units go into each point, and which muscles are deliberately left alone.

That last part is the one that gets missed. The muscles of the upper face work against each other: some pull the brow down, one pulls it up. Treat the whole forehead evenly without accounting for that balance and you get a smooth forehead with heavy, dropped brows, which is the look most people are actually afraid of when they say they do not want to look 'done'. We look at your face moving, not just at rest, because that is where the lines are made. We map which muscles are doing the work, discuss how much movement you want to keep, and tell you honestly if filler or something else would serve you better.

Preparation

The area is cleansed. No numbing is usually needed: the needle is very fine and most people describe it as a quick scratch.

Settling in

You go straight back to your day. Stay upright for four hours, skip the gym, alcohol and facials for 24 hours. You will see it start at day 3 and reach its full effect at around day 14.
